Description

ICM is designed to promote stabilization within the community for people who have a history of frequent hospitalization and need multiple contacts per week. People receiving ICM will have contact with a case manager 4-7 times per week and regular treatment team meetings. Focus of services may include connection to resources, development of coping skills, stabilization of symptoms and crisis intervention.
